Statistics show that most accidents are caused by a combination of human factors rather than equipment failures. The current buzz term in aviation safety is "Safety Management System", which is a formal program used to identify and solve any safety gaps within an operation. Many flight departments are implementing the International Standard for Business Aircraft Operations (IS-BAO) which is an attempt to set a safety culture standard that is adoptable to any size organization. Some countries are now requiring IS-BAO; we expect insurance companies to be asking about your IS-BAO status in the next few years.
